In-Network vs. Out-of-Network

In-network providers have contracts with your insurance company and have agreed to accept negotiated rates. Out-of-network providers have no contract, can charge whatever they want, and your insurance may pay little or nothing.
Real-world example
An in-network MRI might cost you $500 (negotiated rate minus insurance payment). The same MRI at an out-of-network facility could cost you $3,000+ because there's no negotiated rate and your insurance may apply a much higher deductible or not cover it at all.
Why this matters for your bill
Staying in-network is one of the single biggest things you can do to control medical costs. Always verify your provider is in-network BEFORE receiving care — and remember that the hospital may be in-network while individual doctors there (like anesthesiologists or radiologists) may not be.
